This page will provide you with a look at the forms that Judy's daycare uses for registration for parents interested in joining our little family.

If you think you may be interested in signing up your child at Judy's Daycare in Bellingham, MA, please contact Judy to find out the current and planned state of openings at the daycare, and to schedule an appointment so that you and she can meet and interview each other.

Forms

The forms that Judy's Daycare uses when starting a new client, and annually to keep information current, provide a start point for information sharing between Judy and the parents of all of her Daycare children.  They include forms for medical, emergency and contact information, as well as descriptive information about the unique needs and delightful attributes of each child in the daycare.  This provides an excellent start point for the ongoing dialog between Judy and parents about their child's progress.

Click on the following links to view each of the forms.  After you schedule an appointment with Judy, if you like, you can print out the forms, fill them out as best as you can,  and bring them along to speed up the registration process.  Don't worry if you don't have all of the information before your first meeting.
